Thank you for our time together

A heartfelt farewell to my patients:

I have retired! It’s difficult to describe the depth of my gratitude to those who have supported me in this journey. It was a dream come true some 25 years ago when I graduated from Medical School. Being a family physician is tough but also incredibly rewarding.

I’ve enjoyed caring for people through all the different seasons of their lives, sharing their joys and sorrows, and building relationships based on mutual trust. I’ve loved watching the pregnant bellies grow, the babies come out, and the families grow. I’m proud to have attended about 1500 births!

I would love to keep up with the families I’ve served over the years. If you are (or were a patient of mine) at Mosaic Family Medicine or Mercy Family Medicine, I have created a private Facebook group for us to share updates as a community. You can find it here. PLEASE note that you must answer the questionnaire in order to be admitted into the group.

With gratitude,
Sandra Minchow-Proffitt, MD

What happens now?

For the time being, you will still have access to your PATIENT PORTAL if you need to access your records. During this time, you can also request to have your records sent to another provider by contacting Mosaic Family Medicine (if your new provider has a records release form, you can fill it out and fax it to us at 314-743-3700).

At some point your records will be transferred to a long-term records storage company. Once the details are ironed out, this page will be updated with details for how to access those.
