Mosaic Family Medicine is always working hard to get linked with more health insurance companies. Below is a list of our approved insurance plans and those which are pending. At this time, Mosaic Family Medicine will not be accepting Medicare (insurance for the elderly).
Please note, this insurance plan list is updated periodically. There are new insurance plans created all the time and sometimes it is difficult to predict if a new plan will be accepted. If you are unsure if Mosaic Family Medicine or Dr. Minchow-Proffitt is a preferred provider with your insurance, please call your insurance plan before your appointment. If you are seen and insurance refuses to pay, then you will be required to pay for the visit. Please let us know if you have any questions about insurance.
