Self-Pay Patients

If you are a self-pay client, or you are being seen by Mosaic Family Medicine prior to the finalization of insurance plan agreements between Mosaic and your insurance company, you will be expected to pay at the time of the visit as a self-pay patient. Note: Even if you have previously been a patient of Dr. Proffitt, legally you must be charged a New Patient Appointment upon your first visit to Mosaic Family Medicine, since it is your first visit to this new practice.

What is the cost if you don’t have health insurance?

A discount of 30% is given to all self-pay patients if paid at the time of service! Here are the usual charges and discount rates for self-pay, for various types of office visits:
